Eligibility
Requirements:
Below
is a list of eligibility requirements for
Subhaulers. In order to qualify as
a candidate for operating as a Superb Transport,
Inc. Subhauler, you must first provide us
with the following:
A photocopy of your valid and current:
- California Driver’s License
- Social Security Card
- Medical Examiner’s Certificate
A current and valid Insurance Certificate
(Certificate/Accord Form & Additional
Endorsement) must be presented prior to
approvale.
The
following must be reflected on the Insurance
Certificate:
- Superb Transport, Inc. must be named
as Additional Insured
- General/Auto Liability coverage ($ 1,000,000)
- Cargo coverage ($ 100,000)
- Trailer Interchange ($ 25,000)
The
following name/address must be reflected
on the Insurance Certificate and Additional
Endorsement:

Drug & Alcohol Testing Program

contact information for the consortium will
usually suffice

Superb Transport, Inc. Drug and Alcohol
